The Chicago Federation of Labor

CFL COVID-19 Vaccine Distribution

Welcome to the Chicago Federation of Labor COVID-19 Vaccination Registration

The CFL is working directly with local unions to distribute codes for access to vaccine appointments. Your local union will contact you when vaccine appointments are available. Please do not contact the CFL directly for vaccine access - your local will contact you. Once you have received a code from your local, use the link at the bottom of this page to schedule an appointment (if available).

• If you cannot make your appointment time, you must cancel your appointment. You are not allowed to give your timeslot away. The name on the registration must match the name of the person coming for the vaccination.

• Proof of Union Membership - You MUST SHOW PROOF of union membership when you arrive for your appointment (union card, receipt of dues payment, email invitation direct from the union regarding this vaccination event, etc.) to be allowed into the vaccination site. This site is for union members only. If you are not a union member or cannot prove that you are, you will not be allowed to enter.
• Government ID. The name on the ID must match the name on the registration. (If you don't have a government ID, bring another form of ID.)
• Your medical/health insurance card and your prescription card (if applicable).
• Informed Consent Form: CLICK HERE
o Print the linked consent form and bring it with you. Forms will be available on site if you cannot print ahead of time.

To limit the number of people in the building:

• Only those being vaccinated will be allowed to enter the building. Anyone needing assistance getting around will be allowed to bring one additional person if necessary.
• DO NOT ARRIVE MORE THAN TEN MINUTES PRIOR TO YOUR SCHEDULED TIME. You will not be allowed into the building until your scheduled appointment.

**NO ONE UNDER THE AGE OF 18 will be allowed inside the vaccination site. If you have children, please find other arrangements before coming to the vaccination site.**
